You have lost your student identification card. This has created some problems for you. Write a letter to the director of student services. I missed my ID card yesterday when I was coming on my way to the University, I noticed that it wasn't with me the moment I wanted to enter the University, I faced a real problem from the gate , as well as I had been forbidden from accessing the Library, I couldn't progress in my research. And not only that , to attend my chemistry laboratory I had to bring a permission from the dean, the problem that the permission I had was valid for only 48 hours, until I can get my new identification card. As I need the card in every step in my college, most of my Business is hanging for now , I wouldn't be able to review some references or borrow them from the library, moreover , I have to submit my final research within one week , as the deadline is about to due , every day has a great value to me . Other problem , the missing laboratory classes are very hard to be substituted.  For all above reasons ,I want to apply for new card , hopefully, as soon as possible, in order to attend my classes and be able to finish my research in time , as you know the temporal permission would be expired by the end to today , I'm wondering if any official form should be filled .
