write a referral letter to a Occupational Therapist
I am writing to refer Mr Graham, a 43-year-old forklift driver with chronic low back
pain
for workplace assessment.
Mr. Jones is my regular patient at the clinic. Today he came for the evaluation of his back pain
that started 3 months back after lifting a heavy box off the floor at work
. At that time, I advised him to take one month off work
and I requested physiotherapy. In his second follow-up, he still complained of back pain
and stiffness.
He has been managed during these 3 months by resting and regular physiotherapy. His walking ability without pain
increased slowly and gradually and he is better, but still, he mentioned significant tiredness after 30 minutes of walking.
He is bored and discouraged. he wants to join to work
. I let him return if he does not have any lifting and he has a regular break.
I would appreciate it if you could visit his workplace and adjust him for returning to work
after the recent back injury.
Yours sincerely
