Dear Ms Rudik, I would like to introduce Ms Mary Bell, who has been treated in our facility due to unstable (D)diabetes Mellitus and small infected left foot ulcer. She will be needing your medical and social assistance. Ms Bell has been diagnosed with non-insullin-dependent diabetes (since)for 15 years. She was prescribed with hypoglycemic medications: Glimel, 5 mg daily and Metformin, 800 mg thrice daily.
