You work in an international company. You need to take some time off work. Write a letter to your employer. In your letter - explain why you need this time off - give details of when you want this time off - say who can do your work when you are away.

Dear Mr Smith, I am writing in regards to request urgent 2 weeks time off starting from 2nd April because of my mother's medical condition. Due to the annual medical check-up, a suspicious turma in the worm has been found , and she was instructed to hospitalize to do a major operation. Since there is nobody taking care of her, I would have to assist during her operation and recovery time. According to her specialist, she has to bed rest for 2 weeks after surgery.
Thus
, I will have to accompany her until she can do her normal activity and would like to take time off from 2nd April until 15 April. During my absence, my colleague, Harry Smith, will take over my responsibility. He is the one who has been doing the project together with me for almost 1 year , and he is aware of my daily activities and the status of current ongoing projects. If there is any urgent issue, he can contact my personal contact number as well. I hope you can approve my leaves and am sorry for any inconvenience caused. Yours faithfully Jame Luke
