Thanks for seeing your previous patient, Ms. Bethany Tailor, a 35-year-old schizophrenic patient, who was admitted with decompensation. She is ready for discharge today and ongoing care is required.
Ms. Tailor came for admission to our hospital on 01/03/2018,
while
she was agitated and aggressive and she claimed hearing an auditory command and hallucination to harm herself. She had visual hallucinations and delusions as well. The antipsychotic medication, risperidone, was started and after 10 days she stopped reporting auditory and visual hallucinations.
She is a polysubstance abuser, mainly cocaine and alcohol. The last
usage of cocaine was 2 days before admission. She is a non-compliant patient and takes her medications irregularly. As you know she has hypertension and hypothyroidism and she is on treatment.
On discharge time her insight is good. She has a fair judgment. No signs and symptoms of psychosis are there.
I would appreciate it if you could continue your follow-up with her, especially needs more concentration on the major aspects of her decompensation, medication compliance, and substance abuse.
