Your patient Ms Bethany Tailor,35 years old unemployed female, self-admitted with decompensated schizophrenia in the psychiatric ward of St. Mary’s Hospital on first of March.
Ms Tailor was agitated and aggressive on admission day. She reported visual and auditory hallucinations. And
also
thought blocking and latency. A very important part of her history is noncompliance with her medications. she is also
a polysubstance abuser more with cocaine and alcohol. It may be helpful to mention the last
use of cocaine was 2-3 days before admission.
During the hospitalization, previous antipsychotic medications were started again. She had a good response to treatment and finally
visual and auditory hallucinations were ceased.
Today she is ready for discharge. she has a good insight and her judgment is fair. She does not report any delusions or hallucinations. I recommended her to continue risperidone 1 milligram twice a day and 1 milligram in case of any agitation or psychosis. I emphasize that decompensation may happen in case of using medications irregularly or abusing substances.
