Dr Martins General Practice 29 Cowslip Avenue Berkeley 10 April 2018 Re: Robert
Smithson
, 22 years old Dear Dr Martins, I am writing regarding Mr
Smithson
, who was admitted four days ago into our emergency department with partial-thickness burns on his chest and neck, followed by respiratory distress. He is now ready to be discharged and will require nursing management and evaluation. Mr
Smithson
’s nursing care mostly consisted of monitoring his haemodynamic status, irrigation of wounds with tap water as well
as
a close and consistent assessment of his respiratory system. Correct word choice

, he was constantly checked for pulmonary sounds through auscultation, but no sounds of stridors, hoarseness, or wheezing were presented. Despite his successful hemodynamic evolution, Mr
Smithson
maintained the head and torso elevated, high flow oxygen and IV fluids throughout his hospitalisation. On discharge assessment, Mr
Smithson
’s breathing was within the normality, with no signs of infection, or significant alteration on his blood test results noticed, but the consultant’s report is still pending. Regarding his prescriptions, he is now on Naproxen and Tramadol for reduction of inflammation and pain, and started on Gabapentin 300mg, to be taken at night time. The patient was educated about the use of suncream, massage techniques to reduce skin dryness, a healthy diet and breathing exercises to promote lung expansion and fluid drainage. It would be highly appreciated if you could promote nursing management care for his wound and daily auscultation of his lungs. Please reinforce avoidance of any contact sports, but encouragement for speed of the recovery is crucial as Mr
Smithson
is a very active person who enjoys swimming. Mr
Smithson
’s next appointment will be at Burns Unit Outpatient on 17 April 2018. If you have any queries, please do not hesitate to contact me. Sincerely, NurseSubmitted by talitamartinsgo on
