Ms Jane Gold Headnurse Primrose Nursing Home #3 Blackwood Street Oldtown May 15, 2021 RE: Mr George Gale, DOB: April 24, 1936 Dear Ms Gold, I am writing with regard to the discharge of Mr George Gale, who presented in Oldtown hospital due to a urinary tract infection that possibility cause of his fall. He requires additional care upon discharge. On the day of admission May 9, he reported that he experienced a fall, subsequently, a head strike happened. Therefore, notable confusion and disorientation were seen. Additionally, his vital signs were elevated,and a urinalysis was ordered and which reveals he has a high bacterial infection. Within 4 days in the hospital he was treated with amoxicillin intravenously: 750mg every 8 hours. Afterwards, his vitals returned to normal. His medical record shows that he was diagnosed with Osteoarthritis and he takes paracetamol for it. He has had hypertension since 2009 and takes Felodipine 5mg to control his blood pressure levels.
