This idea hinges mainly on work in Figures 2,3 and 4. In barcoded KCL-22 cells (Figure 2b-f), clones that grow with ABL001 are not the ones that grow with nilotinib, so each drug leaves a different resistance pool. KCL-22 xenografts on single-agent ABL001 or nilotinib show an early drop in tumor size and then a return of growth with resistance (Figure 3a). With the ABL001-nilotinib pair from the start (Figure 3b), tumor size falls and does not return after treatment stops. In one CML patient, ABL001 gives a clear response and later the disease comes back with myristoyl-pocket mutations, the same kind of resistance as in the preclinical work (Figure 4).
2.The authors test this idea in a few linked ways. They first show how ABL001 works as an allosteric inhibitor with crystal structures, NMR, and tests of ABL1 activity and cell growth in BCR-ABL1- dependent cells. They then track resistance with a barcoded KCL-22 population treated with ABL001 or nilotinib, to see which pre-existing clones grow out under each drug. Next, they treat mouse CML xenografts with ABL001 or nilotinib alone and with ABL001-nilotinib pair and follow tumour shrinkage and relapse after treatment stops. They also look at early phase I data for ABL001 in patients with CML to confirm that the drug has clinical activity and to flag resistance in humans.
1.In this paper, the authors test whether an allosteric drug, with a catalytic-site tyrosine kinase inhibitor (TKI) can control chronic myeloid leukemia (CML) better than one drug on its own. ABL001 binds in the myristoyl pocket of BCR-ABL1 and turns the kinase off. Nilotinib sits in the catalytic site. Each drug misses a different set of BCR-ABL1 mutants, so one drug alone can still leave small group of cells that still grow. In mouse CML xenografts, the two drugs together shrank tumours, and they didn’t grow back after treatment stopped. The ABL001-nilotinib pair left little room for resistance in this setting.
