District Nurse District Nursing Centre Lake Park 12 September 2019 Re: Mr Andrew O'Connor, 74-year-old Dear Sir/Madam, I am writing in regards to Mr O'Connor who suffered from cerebrovascular accident which resulted to right-sided paralysis. He will be discharged in 3 days. He requires your continued management. During hospitalization, he underwent an intensive physiotherapy and occupational therapy, but yielded slow progress and still needs aid with grooming, hygiene and getting in and out of the bed. However, he can eat by himself and ambulates slowly with the use of frame. In view of his present condition, the family were advised nursing home placement but refused. After home assessment by an OT, modifications were made which includes installation of bed rails and hand rails in bathroom and toilet.
