Dr Martins 29 Cowslip Avenue Berkeley 10 April 2018 Re: Mr Robert Smithson, 22 years old Dear Dr Martins, I am writing regarding Mr Robert Smithson’s discharge tomorrow. He was treated for partial thickness burns on the neck and chest accompanied by respiratory distress and he requires continuity of care. On 06 April, he was presented in our facility due to 18 percent TBSA burns. A series of laboratory tests were done and revealed normal. In addition, his arterial blood gas and cardiac monitoring showed a normal range. Although there was no evidence of wheezing, he was then monitored for signs of respiratory problems. He had a urinary catheter and was been monitored hourly. Consequently, he was observed for fowler’s position. Overall, Mr Smithson was stable and was commence on oral analgesics, gabapentin and tetanus prophylaxis.
